Integrated into our market-leading PharmOutcomes solution, Digital Gateway provides a self-triage option for patients who present at your pharmacy with one of seven common conditions.
Patients simply scan a QR code and complete a brief screening assessment to determine their eligibility for a Pharmacy First pathway. This data is then automatically transferred into PharmOutcomes, pre-populating the relevant service template ready for the consultation.
If a patient is not suitable for a Pharmacy First pathway, Digital Gateway will direct them to the most appropriate care and advise they speak to the pharmacy team, where appropriate.
