From hospital to home: how virtual wards are transforming care at East Cheshire NHS Trust
Virtual wards are transforming care delivery, enabling patients to receive hospital-level care at home, supported by clinical teams and remote monitoring.
At East Cheshire NHS Trust, over 1,300 patients have been treated through their virtual ward service. By enabling early discharge and preventing unnecessary hospital admissions, the Trust is increasing acute bed availability — directly supporting efforts to reduce the elective care backlog and helping patients recover more comfortably at home.
With digital tools like EMIS Web Bed Management, their teams can seamlessly track patients across settings, step care up or down as needed, and improve outcomes — all while easing pressure on hospitals and ensuring patients get the care they need.
"Our consultants were gobsmacked when they saw how they could track all their patients in one place. During multidisciplinary team meetings, everyone can see the same information on big screens, making care coordination much more effective." - Liam Radcliffe, Digital Applications Manager at East Cheshire NHS Trust
