A risk stratification tool which has been developed to integrate with your EMIS Web clinical system

GPs need to undertake regular risk profiling and risk stratification of their patients. NHS England's Enhanced Service rewards practices for the pro-active case management of patients classed as vulnerable or at risk of an unplanned admission and to identify their top two per cent of at risk patients via a risk stratification tool.

Our risk stratification solution is a fully integrated tool which uses the peer-reviewed and clinically validated QAdmissions algorithm. The embedded tool will search on 29 different risk factors within the patients record, producing an individual risk score which supports the clinician to case-find patients and create a personalised care plan preventing an unplanned hospital admission.
