An end-to-end social prescribing tool

Social Rx supports Primary Care Networks, the Voluntary and Community Sector, Clinical Commissioning Groups and Local Authorities with a complete social prescribing software solution.

Social Rx enables the tracking of patients and other users as they flow through your social prescribing pathway. Link workers can use Social Rx to manage their social prescribing caseload, schedule appointments and record case notes, action plans, assessments as well as arranging referrals to local community services through the internal directory of services.

Progress monitoring and easy reporting

The product includes a range of real-time dashboards and reports to provide managers and commissioners with views of population level data. These can be used to understand capacity and demand challenges to help drive social prescribing improvements.

The product is fully interoperable with EMIS Web so that GP’s can easily refer patients into social prescribing without leaving the clinical system, whilst social prescribing data is also available to view from within EMIS Web.
