DOC@HOME is easy to both implement and use in a number of healthcare environments, including care homes, primary care settings, patients’ homes and hospital-to-home.
Docobo has been providing healthcare services to improve care homes for many years, supporting the Enhanced Health in Care Homes (EHCH) programme, which specifies nomination of GPs to care homes and improves responsiveness and care.
Docobo also provides remote monitoring for long-term conditions - providing proactive care for single and multiple conditions, which reduces unplanned admissions, enhances care for patients and reduces clinical workload.
Information is sent to DOC@HOME via a choice of patient-accessible devices and methods such as tablets, smartphones, web access and messaging. All devices connect to the central DOC@HOME clinical system which is an accredited medical device. The wide range of options helps to ensure no patient is digitally excluded.
In care homes, monitoring all residents in this way enables clinicians to develop a baseline for each person’s ‘normal’ condition. The system also caters for residents with long-term conditions and carers can record observations for everyone, regardless of whether they have underlying conditions or not. This results in substantial time savings for GPs and carers.
