A community rehabilitation team in Cheshire is offering more coordinated care to vulnerable elderly patients.

The team at the Cheshire and Wirral Partnership NHS Foundation Trust provide vital care to ensure that approximately 750 patients – mostly frail, elderly people aged over 65, who have suffered strokes or falls – remain independent in their own homes and avoid admission to hospital.

Before switching to EMIS Web®, the 45-strong team of physiotherapists, occupational therapists and therapy assistants used paper patient records. Now they all work from a common electronic patient record via EMIS Web, which has helped them to improve communication and workload planning. It is hoped EMIS Web will drive up efficiencies in patient care.

“I would highly recommend EMIS Web. It has totally changed the way we work – we are in the 21st Century now,” said administration manager Mandi Harvey. “For the first time, all the information about patients’ care is there in front of us. It means that we are able to deal with queries from patients and from our team who are out on the road, without having to make endless phone calls. It has made team working much more efficient.”

“Caseload management is much improved. We can now very easily see who is providing what for patients, and that means their care is much better coordinated and effective,” said Kate Sharp, community rehabilitation team leader. Management reporting on team activity has also improved since the team switched to EMIS Web.

“It is much more intuitive. I just press a button and the report is there for me – it is simple to do,” said Kate. “We can split the data down into individual therapists’ visits and provide a more detailed picture of care. It is helping us to plan and provide better services to patients because we have a far clearer idea of what we are doing day-to-day.”
