We’re pleased to announce that EMIS has been awarded full rollout approval for NHS Digital’s Transfer of Care Initiative.

This allows GP practices to receive electronic discharges and outpatient letters from hospitals straight into Workflow Manager, subject to the hospital systems ability to construct correspondence as Fast Healthcare Interoperability Resources (FHIR) documents.

EMIS Web is now enabled to receive and process all four Transfer of Care use cases following the successful First of Type work with secondary care organisations Dorset County Hospital, Leeds Teaching Hospitals, Oxford Health and Devon Partnership, who have been working with over 15 Practices within local and neighbouring CCGs.

Transfer of care covers four types of hospital message types:

  • Inpatient and Day Case Discharge
  • Accident and Emergency Care Discharge
  • Outpatient Letters
  • Mental Health Discharge

Message Exchange for Social Care and Health (MESH) mailboxes have been configured at all of EMIS’ England GP practices to receive Transfer of Care messages.

Any practice wishing to receive discharges and outpatient letters via this new functionality should liaise with their CCG and local Hospital Trusts to ensure hospital correspondence systems can send FHIR messages via MESH. Interested parties can liaise with NHS Digital on the sender-side assurance process.
