Rochdale Health Alliance (RHA), has been using EMIS Web® to drive meaningful change across its 36 practices. With over 250,000 patients in the area, the goal is clear - make sure everyone receives the right care, in the right place, at the right time.
Using EMIS Web PCN Hub, RHA has been able to reimagine their triage system with improved signposting, ensuring that patients are directed to the most appropriate service, whether that's a GP, pharmacist, nurse, or social prescriber.

The integrated nature of the system means that even when patients are seen by different teams, such as community nurses, care home staff, or local pharmacists, their records are always up to date and accessible because they all use EMIS Web. By unlocking and analysing the data within EMIS Web, RHA can now track what’s working and plan targeted health campaigns for the future. It also means they can clearly demonstrate their impact to commissioners, evidencing their achievements.
