Joined-up thinking

Patients Know Best (PKB) connects information from primary, secondary, social, and mental health care providers, to create a single, unified copy of patient data. This helps reduce duplication, increases capacity, and supports a patient’s multidisciplinary care needs.

Patients can access their health details online to assist self-management of their wellbeing. Using an integrated care record, healthcare professionals can access the same data to support clinical decision making, manage patient care more effectively, and promote independence.

Patient benefits

Patients can access a range of features including appointment letters, test results, messaging, care plans, and tailored resources created by their healthcare teams. Using a granular consent model, they can also upload real-time data - including vitals - from wearable and monitoring devices and share this with all the teams involved in their care.

Wide integration

PKB was the first personal health record to integrate with the NHS App. It extends the app’s features by sharing GP data so patients can access full health and care records, and functionality, direct from the national NHS interface. PKB is part of the national Wayfinder programme backing the NHS’s elective recovery plans.

Integrated-data-feeds from EMIS Web provide the PKB public health record with demographic and clinical information. Professionals can also use a single-sign-on from EMIS Web into PKB.
