Delivering multidisciplinary care closer to home with EMIS Mobile

Coordinating end-of-life care can be complicated at the best of times. But by using our technology, healthcare professionals are able to work together to support people’s physical, social and wider holistic requirements in the place of their choosing.

By aiming to care for people who have advanced illnesses at home – and by giving those people the option to die at home if they wish – hospice at home services do vital work. Their services are an integral part of end-of-life care and bring the skills, mentality and practical care associated with hospices into spaces that are familiar and comfortable to patients.

82% of people want to die at home

50% of people die in a hospital

How to achieve gold standard hospice at home services

But that doesn’t mean the care they deliver isn’t without its challenges. Ian Bailey, district nurse, Queen’s nurse and our clinical design director, talks about these complexities and how technology can help. He argues that technology can not only join up the care provided by many services, but can also support people’s end-of-life wishes by allowing hospice teams to care for those people in their own homes.

Read Ian’s article here to learn how hospice at services are not only better for patients, but could save the NHS a significant cost too.

Hospices could care for 50,000 people that die in hospitals

This would result in a 20% reduction in hospital deaths

And could generate £80 million in savings for the NHS

How EMIS-X Mobile can support better end-of-life care

Being able to securely share records can make the world of difference when trying to coordinate and join up the multiple services that can be involved in end-of-life care.

Our clinical IT system, EMIS Web, provides quick and easy access to real-time patient information so that you can make better, safer decisions. By then using EMIS-X Mobile to access the core elements of EMIS Web via your mobile device, you’re supported to:

  • Deliver informed and integrated care – view patients’ medical records, access supporting documents like tests results, and edit and share information.
  • Effectively organise your day – view a map of appointment locations to plan journeys.
  • Efficiently update notes – easily enter information like new medications into straightforward templates and use your device’s voice recognition capabilities.
  • Always stay informed – download patient records to view them without an internet connection.

With clinicians always able to access the information they need, they can enter patients’ homes fully prepared, and able to understand a patient’s situation and what other care they're receiving. It means people at the end of their lives and those around them don’t need to repeat themselves or answer difficult questions they might not know the answer to.

EMIS-X Mobile in action

For those who want to see the transformative effect of technology in action, look no further than East Cheshire Hospice.

Before we even go into a patient’s home, we’ve got all that past history already at our fingertips.”

Suzanne Elliott

Sister, East Cheshire Hospice

Having launched their hospice at home service in 2018, they’re using EMIS-X Mobile to support people to both stay at home as long as possible and die at home if that’s their preferred place of choice.
