Connected care in action: How Blackpool Teaching Hospitals NHS Foundation Trust transformed patient care
Connected care systems have the power to transform patient outcomes, something Blackpool Teaching Hospitals NHS Foundation Trust experienced firsthand. By adopting EMIS Web®, clinicians across mental health, community and primary care can now access and share critical patient information within a single system for the first time. This unified view is breaking down silos and is enabling faster, more informed decisions that truely improve patient care.
The change has delivered measurable benefits, freeing up clinical time previously spent chasing information, reducing duplication and cutting delays in treatment. Clinicians can now focus on what matters most, patient care.
"This has been the best project I've ever had the pleasure to be involved with. I'll use this as an example of something that works incredibly well in the NHS." — Kim Webster, Blackpool Teaching Hospitals NHS Foundation Trust

Making waiting safer and fairer
University Hospitals Plymouth NHS Trust is tackling the elective care backlog head-on. Working with Optum, they introduced a proactive “waiting well” programme powered by PHM Pathfinder Analytics to analyse population data. By segmenting over 40,000 patients and identifying those most at risk, including individuals with learning disabilities, they successfully reduced waiting times.
