Our End of Life Datasets provide a way for healthcare professionals to ensure that everyone involved in a patient's care can easily view targeted, real-time medical information about a patient's end of life care plan. This leads more informed decisions so that patients wishes are met towards the end of life. Our End of Life Datasets available via Medical Interoperability Gateway (MIG®) include:
- Electronic Palliative Care Co-ordination Systems (EPaCCs)
- EPaCCs Frailty
- Supportive Care Planning.
Our End of Life Datasets can be presented in a HTML view or structured XML data, depending on the compatibility with your GP system provider.
EPaCCs has provided a platform for end of life decisions and conversations to be documented and shared in such a way that clinicians in both the community and acute settings can access it.
In addition to our End of Life Datasets, we provide a range of health and social care datasets that offer additional feeds of real-time patient information via Medical Interoperability Gateway (MIG).
