Manage your patients at risk of unplanned hospital admission
Our risk stratification solution is a fully integrated tool which uses the peer-reviewed and clinically validated QAdmissions algorithm. The embedded tool will search on 29 different risk factors within the patients record, producing an individual risk score which supports the clinician to case-find patients and create a personalised care plan preventing an unplanned hospital admission.

Enterprise Search & Reports
Identify the right people at the right time to improve care
Throughout the UK, NHS organisations are benefiting from instant access to GP data using EMIS Web Enterprise Search & Reports.
CCGs, Public Health, GP Federations and Health Boards can perform centralised real time searches on the data held by any EMIS Health practice.
Fast and efficient
It’s a fast, efficient way to see up to the minute health information that can be used in a number of ways to benefit patient care, for example:
- improving call and recall rates such as childhood vaccinations. For NHS Tower Hamlets this has improved uptake of MMR second dose from just over 80 per cent to almost 95 per cent
- ensuring accuracy and consistency of medical records – making sure there is a diagnosis present if medication for the disease or problem has been recorded
- gathering data to improve the quality of services and monitor patient care to assess initiatives
- to assess potential cardiovascular risk when used in combination with QRisk – the cardiovascular risk score developed by the University of Nottingham in partnership with EMIS Health.
The entire process saves time because data is collected remotely and is available immediately, without relying on visits to practices to collect information.
